Cervical dystonia, also known as spasmodic torticollis, is a painful neurological movement disorder that can cause involuntary neck spasms, abnormal head positioning, and significant disruption to daily life. In this article, you’ll learn about the causes and symptoms of cervical dystonia, how it is diagnosed, and the advanced treatment options available to relieve muscle contractions and restore comfort.

If you are experiencing persistent neck twisting, head tremors, or unexplained neck pain, specialized neurological care can make a meaningful difference. At New York Neurology Associates in New York, NY, our board-certified neurologists provide precise diagnoses and individualized treatment plans designed to reduce symptoms and improve quality of life.

What is Cervical Dystonia?

Cervical dystonia, also known as spasmodic torticollis, is a neurological movement disorder that causes involuntary contractions of the neck muscles. These muscle spasms lead to abnormal head positions, tremors, and pain that can interfere with daily activities and quality of life.

The exact cause of cervical dystonia is not fully understood. In many cases, it may involve dysregulation in the basal ganglia, a region of the brain that helps control movement. Some cases are idiopathic (no known cause), while others may develop after neck injury, medication exposure, or genetic predisposition.

Symptoms of Cervical Dystonia

Cervical dystonia symptoms vary in intensity and pattern, but may include:

  • Involuntary twisting or turning of the head to one side (torticollis)
  • Forward flexion (anterocollis) or backward extension (retrocollis) of the neck
Tremor or shaking of the head
  • Muscle tightness, stiffness, or pain in the neck and shoulders
  • Head position that worsens with stress or fatigue and improves during sleep

Symptoms may begin gradually and worsen over time, though in some individuals they stabilize or improve with appropriate treatment.

Evaluation and Diagnosis

Accurate diagnosis is key to effective management. At New York Neurology Associates, our neurologists perform a detailed assessment that includes:

  • Evaluation of neck posture, muscle tone, tremor pattern, and range of motion.
  • Identifying potential contributing factors such as prior injury, medications, or family history of dystonia.
  • Electromyography (EMG) is used to confirm abnormal muscle activity and guide precise treatment planning.
  • Imaging (MRI or CT) is necessary to rule out structural causes of secondary dystonia.

Through this comprehensive approach, we differentiate primary (idiopathic) dystonia from secondary causes and tailor treatment accordingly.

Treatment Options

Cervical dystonia is a chronic condition, but with proper management, many patients experience significant improvement in symptoms and daily function.

Our specialists provide a full range of therapies, including the following:

1. Botulinum Toxin Injections (Botox®, Dysport®, Xeomin®, Myobloc®)

Botulinum toxin injections are the gold standard treatment for cervical dystonia. Small doses are injected directly into the overactive neck muscles to reduce involuntary contractions and improve posture.

Key features of botulinum toxin treatment include the following:

  • Injections are often EMG-guided for accuracy.
  • Effects typically begin within days and last about 3–4 months.
  • Injections can be repeated safely as part of an ongoing care plan.

2. Oral Medications

Oral medications available include the following:

  • Muscle relaxants (e.g., baclofen)
  • Anticholinergic agents (e.g., trihexyphenidyl)
  • Benzodiazepines (e.g., clonazepam)

3. Physical Therapy and Supportive Care

Physical therapy can help maintain flexibility, improve neck control, and reduce pain.

4. Referral for Advanced Therapies

For severe or treatment-resistant cases, we coordinate care with regional specialists to consider deep-brain stimulation (DBS) or other emerging treatments.
